We are in Network with Anthem, Aetna, United Behavioral Health, UMR, Tri-care, Medicare, and Anthem Medicaid.

As a curtsey, if the medical necessity requirement has been met, we will directly bill all in-network insurance plans. However, you are responsible for any deductibles and co-payments. You are responsible for contacting your insurance company and verifying your in-network verses out-of-network status, and knowing your deductibles and co-payments prior to the start of treatment.

 

As a policy, you are required to keep a credit card on file which will be billed at the end of each session.

If your insurance is Out-of-Network, you will be responsible for the full cost of services, and will be provided with a Super Bill to send to your insurance for possible reimbursement. Our rates are as follows: 
 

• Initial Intake Assessment (60 Minutes)- $200.00

• Individual Therapy (45 Minutes) - $140.00

• Individual Therapy (60 Minutes) - $160.00

• Couples Therapy (60 Minutes)- $160.00

• Co-Parenting Education (6 weeks)- $320.00 per person

• Co-Parenting Counseling (60 Minutes) - $160.00

• Family Counseling (90 Minutes)- $240.00
