Eligibility Criteria

Patient must have three or more impairments to ADL’s Patient must have income that is less than 300% of the Federal Poverty Level Patient must have $2000.00 or less in countable assets

If Patient is over the income limit but under the asset limit, refer them to Tish at Beacon of Light LLC. (219) 292-7555

If the Patient is over the income limit, and has less than $50,000.00 in assets, refer them to Tish at

Beacon of Light LLC. (219) 292-7555

If the Patient is over the income limit, and has more than $50,000.00 in assets, refer them to a local Estate Planning and Elder Law Attorney

How to Send a Referral to NWICA

Fax the following information to NWICA, (219) 794- 1860, ATTN: I & A

· Name and Contact Information of the Nurse Case Manager

· Patient Name

· DOB

· Indicate the 3 or more ADLS

· List of current medications

· Phone #

· Patient’s Address

· Social Security Number

· Indicate if the Patient is Home Bound

· Medical Insurance

· Policy #

· Caregivers Name

· Caregiver’s Phone #

NWICA has 48 hours to call the Patient. When they call, they are obligated to make sure the Patients have exhausted all other resources before offering grant services. It is important to prepare Patient’s that they will need to be persistent in perusing available services. Patient’s may get confused and decide not to apply for benefits. Tell the Patients if they have not heard from NWICA within 7 days, to let you know.
